Applying for compensation – Forms overview
Victims of violent crimes, their relatives, and surviving relatives can apply for financial compensation from the Schadefonds. Below you will find all available application forms, organized by applicant type. Each form includes instructions and required documentation.
Required documents
Depending on your situation, you may need to add:
- Police report or case number
- Medical reports or treatment summaries
- A court verdict, if a lawsuit has taken place
If you do not yet have all documents, you can often still submit the form and send additional information later.

Victim
Schadefonds Geweldsmisdrijven is there for anyone who is a victim of a violent or sexual crime and has suffered serious physical or psychological problems as a result.
Applying as a victim
Use this form if you are the victim of a violent crime and you want to apply for compensation yourself.

You are filling in this form for someone else. You are doing this as a parent, guardian or curator.

You are filling in this form for someone else. The applicant has given you official permission (authorised you) to fill in this form. So, you are the authorised representative.

Has anyone in your family been a victim of a violent or sexual crime? And does that person have physical or psychological problems as a result that might never go away? This also affects your life. Therefore you can apply for compensation on your own behalf.
You are a close relative if, for example, you are the spouse, (registered) partner, parent, child or (half) brother / (half) sister of the victim.

Use this form if you are a relative of a victim and you want to apply for compensation for the impact the crime has had on you.
